CARIHI believes that harm to patient is predominantly as a result of system level failings. We work to build an understanding of safety at a system level.
Our focus on patient safety includes:

- Promoting the measurement of harm and its antecedent factors.
- Helping leadership understand and address safety issues
- Helping managers and frontline workers understand and mitigate hazards in the healthcare environment.
